Assisted Outpatient Treatment Case Manager (Morehead, KY)

Salary - $35K - $40K
Educational Requirements - Bachelor's degree in Human Services with experience or equivalent experiences in a related field. A minimum of two years' experience is preferred.
Certification/License Requirement - TCM Certification preferred but will need after six months of hire.
Experience Required - Experience in Behavioral Health setting is a plus. Case managers will display competency in working with computers and electronic devices necessary for completing job.
Duties and responsibilities - Staff person would work with adults and elderly consumers designated as Seriously Mentally Ill (SMI) and in need of services as well as court ordered consumers in the Assist Outpatient Treatment (AOT) program. The disability served will be any AMI mental health diagnosis addressed in the current Diagnostic and Statistical Manual or Mental disorders. Provide case management services to assigned consumers and adhere to all Pathways policies and procedures.
