Education and/or Licensing Requirements:  
  • This position requires a Bachelor’s Degree in a human services related field.
  • One year of experience providing TCM services to children/youth with SED and their families.
  • Willingness and ability to complete HFW Facilitator training.
  • An understanding of and experience with different child-serving systems, including education, behavioral health child welfare, juvenile justice, health and others.
  • Experience working with and a genuine respect for families of children/youth who have complex behavioral health needs and a firm commitment to empowering families.
  • Access to reliable transportation and a valid driver's license. 
 
Duties/Responsibilities:  The High-Fidelity Wraparound Facilitator (HFW) will use a wraparound model to facilitate team development and oversee the process and tasks of the team in order to develop a collaborative and comprehensive plan.  The HFW Facilitator will safeguard these principles and ensure that they are delivered with the highest possible fidelity to the model.  The HFW Facilitator offers consultation and education to all providers regarding the principle of wraparound, monitors progress toward goals, and assures that all necessary data for evaluation are gathered and entered.  They will be responsible for completing the strengths/needs assessment for the individual and family, working in full partnership with the team members to implement a service plan.  Assist in identifying providers of services, natural supports, facilitate team meetings, make regular home visits, monitor all services and maintain fiscal accountability.  All other duties outlined in current job description. 
 
Skills/Knowledge Required: Individual will need a collaborative personality that leads to trust/engagement with others.  Strong interpersonal skills and the ability to work effectively with a wide range community partner in a diverse community.  The ability to manage details and follow through until goals are completed.  Strong verbal and written communication skills.
