Pathways Inc.- Targeted Case Manager (Owingsville, KY)

Salary - $33,000 - $36,000

Educational Requirements:

Bachelor’s Degree in Human Services or related field
TCM Certification (to be obtained post-hire)
Preferred: Minimum of one year of post-graduate experience

Duties/Responsibilities:

Provide case management services to assigned consumers
Adhere to all Pathways policies and procedures

Skills/Knowledge:

Experience in a behavioral health setting is a plus
Competency in using computers and electronic devices required for job duties
