Pathways Inc.- Targeted Case Manager (Owingsville, KY)

Salary Range: $35,000 – $39,375

Educational Requirements:

Bachelor’s Degree in Human Services or related field required
Targeted Case Management (TCM) Certification (must be obtained post-hire)
Required: Minimum of one year of post-graduate experience in a related field

Duties & Responsibilities:

Provide case management services to assigned consumers, ensuring coordination of care and support services
Maintain accurate and timely documentation in accordance with program and regulatory standards
Adhere to all Pathways policies, procedures, and compliance requirements
Collaborate with internal staff, community providers, and other stakeholders to support consumer needs

Skills & Knowledge:

Experience in a behavioral health setting preferred
Strong communication and organizational skills
Competency in using computers and electronic devices necessary to perform job duties
