Salary Range: $35,000 – $39,375

Educational Requirements:

  • Bachelor’s Degree in Human Services or related field required

  • Targeted Case Management (TCM) Certification (must be obtained post-hire)

  • Required: Minimum of one year of post-graduate experience in a related field

Duties & Responsibilities:

  • Provide case management services to assigned consumers, ensuring coordination of care and support services

  • Maintain accurate and timely documentation in accordance with program and regulatory standards

  • Adhere to all Pathways policies, procedures, and compliance requirements

  • Collaborate with internal staff, community providers, and other stakeholders to support consumer needs

Skills & Knowledge:

  • Experience in a behavioral health setting preferred

  • Strong communication and organizational skills

  • Competency in using computers and electronic devices necessary to perform job duties
