Salary: $35,000 - $40,000

Educational Requirements:  
This position requires a BA in Human Services from the approved list located in KAR. 
 
Duties/Responsibilities:  
Targeted Case Managers (TCM) are permitted to serve the following populations as long as they receive the 12- hour Core training and specialized SED, SMI, SUD and/or Co-occurring training within six months of hire or produce a TCM Certificate from a previous place of employment.  The diagnosis of people served should be consistent with the Department of Behavioral Health Checklist for the specified population and meet the criteria for duration, diagnosis and disability.         
TCM services consist of (assessment and re-assessment, linking, advocacy, treatment planning, support and coordination) to those consumers assigned and adhered to all related Policies and Procedures and Department of Behavioral Health and Medicaid guidelines. 908 KAR 2:260, 907 KAR 15:055, 907 KAR 15:060 and 907 KAR 15:065
Supportive Housing Specialist: Will cover activities necessary to assist participants to live with maximum independence in community integrated housing with the goal of increasing community tenure as identified in the participant's Person-Centered Plan.
Other duties as assigned. 

Experience Required:  One year’s experience working with the same population to be served (may include prior employment, practicum or internship). Must be proficient in using an Electronic Medical Record along with other software programs; Microsoft 365 and Excel. Case Managers will display good written and oral communication skills, able to adapt to change, work well in a team environment, work to garner referrals, and provide services that are consistent with Kentucky regulations and Pathways Policies.
