Pathways Inc.- iHOPE Targeted Case Manager (Morehead, KY)

Salary Range: $35,000 - $39,375

Educational Requirements:

Bachelor’s degree in Human Services or related field.

Duties & Responsibilities:

Utilize evidence-based models such as Wraparound, Person-Centered Planning, Family Psychoeducation, Coordinated Specialty Care (CSC), or Care Coordination to support clients.
Facilitate team development and guide the process of creating a collaborative and comprehensive care plan.
Support identification of client goals, needs, and barriers to treatment, while linking families to additional supports and resources as needed.
Facilitate or co-facilitate quarterly team meetings to ensure continuity of care.
Assist the Program Director with outreach, community education, and program fidelity measures.

Skills & Knowledge:

Minimum of one year of post-graduate, paid experience working directly with the target population.
Ability to work effectively with diverse populations and help identify individualized needs.
Completion of required trainings (SED, SMI, iHOPE CSC, and others as directed).
Strong teamwork and collaboration skills.
