Salary Range: $35,000 - $39,375

Educational Requirements:
  • Bachelor’s degree in Human Services or related field.
Duties & Responsibilities:
  • Utilize evidence-based models such as Wraparound, Person-Centered Planning, Family Psychoeducation, Coordinated Specialty Care (CSC), or Care Coordination to support clients.
  • Facilitate team development and guide the process of creating a collaborative and comprehensive care plan.
  • Support identification of client goals, needs, and barriers to treatment, while linking families to additional supports and resources as needed.
  • Facilitate or co-facilitate quarterly team meetings to ensure continuity of care.
  • Assist the Program Director with outreach, community education, and program fidelity measures.
Skills & Knowledge:
  • Minimum of one year of post-graduate, paid experience working directly with the target population.
  • Ability to work effectively with diverse populations and help identify individualized needs.
  • Completion of required trainings (SED, SMI, iHOPE CSC, and others as directed).
  • Strong teamwork and collaboration skills.
