Salary: $35,000
Educational Requirements:

Minimum of a Bachelor’s degree in a Human Services field.
Position Overview:
This is a hybrid position based in the FIVCO region . The Service Coordinator will assure the provision of appropriate services for children and adults with, or at risk for, intellectual and developmental disabilities.
Duties & Responsibilities:

Determine the nature of referrals and assess the need for additional contacts or screenings.

Provide intake into the IDDS system.

Conduct home visits to identify family/individual priorities, resources, and concerns.

Facilitate, develop, review, and monitor person-centered plans on an ongoing basis.

Coordinate, monitor, and ensure timely delivery of services outlined in the person-centered plan.

Maintain thorough documentation and comply with all agency policies.

Perform other duties as assigned in the current job description.
Skills & Knowledge:

Strong working knowledge of community resources and how to access them.
-
Ability to engage effectively with individuals and families regarding developmental disability concerns.

Proficiency in computer systems and electronic documentation.
-
Prior experience working with the IDD population is required.
Travel:

Travel within the FIVCO region required.

Mileage reimbursement provided.
