Position Summary
The Targeted Case Manager for the Kentucky Moms MATR Program provides direct support to pregnant women and mothers up to 60 days postpartum experiencing moderate to severe substance use disorders. This role focuses on assessment, care coordination, and connecting clients with critical community resources to support recovery and stability.
This position serves the FIVCO region and The Journey program, requiring collaboration across multiple community partners.
Educational Requirements
Bachelor’s degree in a behavioral science field required.
Minimum Qualifications- Bachelor’s degree in a social or behavioral science field
- At least one (1) year of experience providing case management services
- Ability to complete required training within six (6) months of hire
Key Responsibilities
- Conduct client outreach, assessments, and individualized service planning
- Provide education on recovery, treatment options, and support services
- Coordinate care and make referrals to appropriate agencies and providers
- Serve as the primary contact for referrals from local health departments
- Collaborate with community partners, including courts, DCBS, law enforcement, and shelters
- Maintain accurate documentation and case records
- Support clients in navigating complex systems and accessing services
Training & Development
- DBH-approved Targeted Case Management training provided within six (6) months of hire
- Ongoing continuing education required to maintain certification
