Salary: $35,000 - $39,375 annually
 

Educational Requirements
  • Bachelor’s Degree in Human Services or a related field required
  • Minimum of one year of post-graduate experience required

Duties & Responsibilities
  • Provide case management services to assigned consumers in accordance with individualized treatment and service plans
  • Coordinate and monitor services to support consumers’ behavioral health, medical, social, and community resource needs
  • Maintain accurate, timely, and current documentation in compliance with agency, state, and federal requirements
  • Collaborate with consumers, families, providers, and community partners to ensure continuity of care
  • Assist consumers with accessing community resources, benefits, and supportive services
  • Participate in treatment team meetings and contribute to care planning efforts
  • Adhere to all Pathways policies, procedures, and confidentiality standards
  • Perform additional duties as assigned within the scope of the position

Skills & Knowledge
  • Experience working in a behavioral health setting preferred
  • Strong communication, organization, and interpersonal skills
  • Ability to manage multiple tasks and maintain accurate documentation
  • Competency in using computers, electronic health records, and other electronic devices required for job duties
  • Ability to work independently while collaborating effectively within a multidisciplinary team
