Position SummaryPathways is seeking a compassionate and dedicated In Bloom Therapist to provide individual and group therapy services through telehealth and within residential settings throughout the FIVCO and Gateway regions. The In Bloom Therapist collaborates with internal and external referral sources to ensure consumers receive comprehensive, quality treatment and wrap-around services.
The therapist will provide psychotherapy to individuals experiencing behavioral health and/or substance use challenges and will be responsible for assessment, diagnosis, treatment planning, and therapeutic intervention. Services may be provided to individuals, families, and groups.
This position requires a professional who is adaptable, collaborative, and committed to delivering evidence-based, consumer-centered care while maintaining compliance with Kentucky regulations and Pathways policies and procedures.
- Provide individual, family, and group therapy services in residential settings and via telehealth.
- Conduct comprehensive behavioral health and substance use assessments.
- Assess, diagnose, and treat mental health and substance use disorders within scope of licensure.
- Develop and implement individualized treatment plans.
- Coordinate care with residential staff, medical providers, case managers, and community partners.
- Collaborate with referral sources to facilitate access to services and continuity of care.
- Maintain accurate, timely, and compliant clinical documentation within the Electronic Medical Record (EMR).
- Monitor consumer progress and adjust treatment interventions as needed.
- Participate in treatment team meetings and clinical supervision as required.
- Respond promptly to appointments, emails, and other communications.
- Adhere to all Pathways policies, procedures, ethical standards, and professional guidelines.
- Support program growth through relationship building and referral development activities.
Must be eligible to practice in the Commonwealth of Kentucky.
- Training and experience in Evidence-Based Practices (EBPs).
- Experience working with individuals experiencing co-occurring mental health and substance use disorders.
- Experience providing services in residential treatment settings and/or telehealth environments.
Required Skills and Competencies
- Strong clinical assessment, diagnostic, and treatment planning skills.
- Knowledge of behavioral health and substance use treatment modalities.
- Proficiency with Electronic Medical Records (EMR).
- Working knowledge of Microsoft 365, Outlook, Word, and Excel.
- Excellent written and verbal communication skills.
- Strong organizational and time-management skills.
- Ability to adapt to change and work independently.
- Ability to collaborate effectively within multidisciplinary teams.
