Duties/Responsibilities:  Will assure the provision of appropriate services for children or adults with or at risk for intellectual disabilities or other developmental disabilities.   Will determine the nature of the referral and the need for further contacts and screening.  Will provide intake into the IDDS system.  Will conduct home visits in order to identify with the family/individual and learn their priorities, resources and concerns.   Will facilitate and participate in the development, review and monitoring of the person-centered plan on an on-going basis.   Will coordinate, monitor and ensure the timely delivery of services identified in the person-centered plan.    All other duties as outlined in current job description.
Skills/Knowledge: Must have working knowledge of community resources and how to access these resources.   Must be able to relate appropriately with parental/individual concerns and attitudes as they pertain to developmental disability problems.  Computer skills required.
