- Ensuring that the medically related emotional and social needs of the resident are met and maintained.
- Developing assessments and written plans of care to identify the needs of each resident.
- Assessing the resident's psychosocial and emotional needs.
- Effectively communicating information concerning a resident’s condition with other departments and government agencies.
- Participating in resident care planning conferences.
- Participating in the resident discharge planning, providing information to residents and families of programs available.
- Serving as a liaison with community agencies to ensure needed ancillary services are available and provided within requested timeframes.
- Making referrals and documenting outcomes.
Qualifications:
- Bachelor’s degree or equivalent experience is required; focused in Social Work or Healthcare administration strongly desired.
- Recent experience in a Social Services role in a nursing home/hospital setting or some other Social Worker role.
- Ability to work in a fast-paced work environment.
- Excellent communication and customer services skills.
- Strong computer skills required due to this facility working solely in an Electronic Medical Records environment.
